I use WTFast (www.wtfast.com) on the Canada East (MTL2) server which puts your route right next to the FFXIV Northeast datacenter for the lowest latency and lag spikes possible. I have no lag issues and extra seconds to dodge now! Pre-WTFast I couldn't do Titan-EX, it was embarrassing!
There are other proxy programs that do the same for different monthly rates, apparently there are even free apps! Take your pick, do your research! Always be careful downloading applications from the internet!
